Severe post-partum bleeding in a 34 year old female

A 34 y/0 suffers severe post-partum bleeding that requires aggressive volume resuscitation and transfusion of 5 units of packed RBC’s.  Seven days after giving birth she has failed to lactate.

What is the reason for this?


Patient has Sheehan’s syndrome  – which is a disorder caused by severe hypotension and lack of bloodflow to the pituitary.  This results in a prolactin deficiency which thus leads to the inability to lactate.
